DURING PHYSICAL  APPOINTMENTS
To send clinical documents
(scripts / referrals / product information / notes)

AFTER PHYSICAL APPOINTMENTS
Follow ups – checking in to see how you are
– you touching base with us
– after nail surgery 

IN BETWEEN PHYSICAL APPOINTMENTS
Monitoring, information, guidance. 

INSTEAD OF PHYSICAL APPOINTMENTS
Follow ups, especially for 
uncomplicated nail surgery
If you are out of town and cannot come in for a physical appointment, and need some guidance.

 CONSENT TO RECEIVE TELEHEALTH SERVICES

This is optional, however recommended for continued care and communication. Itis not to replace physical consultations, but as an adjunct to them. Please read the above. Alternately, you will be informed in clinic if you need to complete this step for your specific concern.
